Well the main reason is connected to the coating on the liquid nano particle. This "coating" called PEG (Polyethylene glycol) was sourced from all over the place, and with various quality issues. It degrades quickly and is unstable unless kept at certain temps.
Now if the vial was stored incorrectly, and say handled incorrectly (ie shaken too hard) the coating would be useless and the LNP destroyed. Some people would have got nothing but a shot of goo in their arm and probably accounts for why some people have zero side effects whilst others were seriously harmed.
I still don't understand how it was reported. Do you really think healthcare workers would say we didnt follow procedure and say it happended that many times?
