I did see some analysis of this where it says that his heel refers to male genitals, most probably wrong but the next verse says "Unto the woman he said, I will greatly multiply thy sorrow and thy conception; in sorrow thou shalt bring forth children; and thy desire shall be to thy husband, and he shall rule over thee"
These passages have always confused me
The word translated "heel" in the Hebrew is ʿāqēḇ and is defined as " a heel (as protuberant); hence, a track; figuratively, the rear (of an army):—heel, (horse-) hoof, last, lier in wait (by mistake for H6120), (foot-) step."

So I think trying to stretch this word to mean the male genitals is exactly that, a stretch. As for the multiplying of sorrow and conception this is referring to "birthing pangs".
