I'm a fan of mullvad myself. They've proven to keep no records, they don't require an email address, they accept cash, bitcoin and monero or you can purchase a subscription card that has only an activation key that you type in with your randomly generated account number.

Mine was acquired with a 10 minute email and a(free) rent a phone number for sms verification to register a burner amazon account and shipped to an Amazon box at a hardware store in another town across the state line.
