People don't easily get close to most high profile CEOs. Either a severe lapse of security (unlikely) or targted/planned like title suggests.
Or maybe just a random 'asylum seeker' at the right place. 72 hour rule or something.
edit: or as u/blacksmith21 suggests, maybe he was doing someone's wife.
It really depends on the threat level. I've seen CEOs of big companies with no security, maybe a driver. I was working in the C-suite for a major defense contractor and the CEO had a detail of 2 with her inside the inner sanctum which went to a detail of 4 as soon as she left the suite.
This guy may have been banging someone's wife and took a bullet for it.
I’ve known lots of CEOs that have no security.
Hell, I met the owner of a multibillion dollar cable company at a sportsbar. Just drinking wine and chatting up everyone.
But the CEO of a health insurance company probably is a more dangerous one than most.
Deny a treatment for somebody’s child that results in the child’s death - not long before a pissed off enough parent is willing to do something about it. One smart enough and motivated enough might very well figure out how to find the CEO.
Hell, my best friend is CEO of a major coal conglomerate and gets the occasional whacky environmentalist email. He has no security.
