I see this as comms:

Nancy broke her hip. She probably has osteoporosis -- brittle bones. Bones are what hold the body together, make it work as a whole. It's the underlying structure.

So...

The structure/system/DS is breaking down and Nancy has "fallen." It's all over for her.
