Anyone have a straighforward document to provide a school requesting an immunization record for admission of my child?
I don't want to include a physician in the process.

Yeah, my wife will be homeschooling our kids. Fuck the public school system. Our girls have a completely blank immunization record, as will the baby that she's currently carrying. Don't like it? Don't care.
Here in Florida I went down to the County Health Dept. and got a waver.. took 10 min.
Ohio has a law allowing religious, medical and philosophical exemptions. In Ohio, the school has an exemption sheet available for any parent wishing to file an exemption. Depending on which state you reside, search for your rights and also any medical freedom groups that can help guide you in the process.
There are very few states that do not have religious exemptions. If you live in one of them, there's not a lot you can do.
For every other state, get the religious exemption, and give a copy to the school.
I'd tell the school district to go and F themselves, but that's just me.
It is a state law situation, Tn has medical or religious exemption form
