Even the best missile defense system cannot be perfect. The Patriot system had a very hard time against Scud missiles during the first Gulf War. The Iron Dome system radars may not have been deployed for 360-degree coverage (it happens if you don't have enough radars and need to prioritize expected axes of attack).
