I think the drug pushers add it to other drugs to fuel the addiction. The victim of addiction may not even realize that fentanyl is in it. If you're an opioid addict, fentanyl is short acting and there are probably better opioids to be addicted to. It is routinely used in anesthesia because of its short duration of action.
