Casualties of the USS Forrestal Fire
134 sailors and airmen died in the fire that broke out aboard the USS Forrestal on 29 July 1967, with 161 others injured. The fire occurred while the aircraft carrier was operating on Yankee Station in the Gulf of Tonkin during the Vietnam War. Many of the deaths occurred below deck, where crew members were caught in berthing compartments when explosions tore through the ship. Flaming jet fuel and debris cascaded into lower levels, trapping and killing numerous sailors in their sleeping quarters.

Cause of the Fire
The fire was triggered by an inadvertently fired Zuni rocket from an F-4B Phantom II jet on the flight deck. The rocket struck the external fuel tank of an A-4E Skyhawk aircraft—piloted by then- >>>Lieutenant Commander John McCain—rupturing the tank and spilling jet fuel. The fuel ignited, starting a fire that rapidly spread. The heat from the fire caused nearby ordnance to explode, including old, unstable 1,000-pound AN-M65A1 "fat boy" bombs filled with heat-sensitive Composition B explosive. These bombs, some over a decade old and poorly stored, were particularly prone to "cooking off" in high temperatures.
The U.S. Navy’s official investigation concluded that a stray electrical signal likely activated the rocket launcher, and that no individual was directly responsible for the incident. Safety lapses in ordnance handling and storage were noted, but no disciplinary actions were taken.
