For most black holes, the accretion disk is in the same direction as the rest of the system (stellar or galactic) so there would not be a change in direction. Somewhat beyond the event horizon, it's just pretty normal orbital mechanics, just a stronger version of neutron star disks. The artist is over-doing it.
