Theta Network has a decentralized streaming platform right now. The data never touches a centralized server owned by anyone. Blockchain is used to reward "miners" doing the transcoding and sharing bandwidth. It doesn't have Video on Demand but it might in the future. (Note: theta.tv is not the decentralized service I speak of, that is their in house twitch type platform curated by them. The decentralized streaming is only on their Edge Node application)
Livepeer is another project working on video distribution with integrated blockchain. Cudos is also another one.
