I've read that it's the result of a humiliation-themed ritual and also read that it's from an injection of some kind (adrenochrome being what I remember). The latter of the two seems far-fetched but when you take in the big picture who the fuck knows what insane, depraved shit these people are into? Just scratching the surface and guessing at some of the rest is enough to scar you mentally and spiritually at the simple knowledge of the level of evil that exists in this world. I always love pointing this out to people and it is a decent chewable red-pill when combined with a demonstration of the common symbolism used by them.
