I looked up the antecubital vein but I am not sure what you mean by "wasn't the only issue". Do you think she was an IV drug user or was receiving treatments for other medical problems?

Yes. The antecubital vein is big and hard to miss. There is one to the side where the big bruise is, looks like a place where it was punctured and leaked.
