If he denied vaccines to his crowds would the Establishment declare all-out war against Trump supporters (calling us murderers)? Is it so bad that Trump actually feels the need to promote the jab?

So they can't go against it. If they did.
The what little support they have would be gone. It basically would mean they took the jab for nothing.
