I will say this, there is an episode of sliders about a pandemic disease named the Q. Not saying this is anything meaningful to this book, just pointing it out.
I didn't read this book but just read a few reviews. One of the reader is talking about a Q island in the book (maybe Manhattan). I think it means Quarantine Island and that could explain the title of the book. I'm not sure of course but I think that's it.
