Probably too scared to come out about it and/or censored, as with many others around the world. Plus, Japan is very big on conformity, so they don't want to risk coming out with an unpopular opinion (from the perspective of global elites) or as a nail sticking out in the international community. I know Taiwan's doctors are very attentive and thorough, so I thought when the covid numbers went up over there (a couple months ago) the doctors would be like "Oh. HCQ works" and the whole world would know... But nope, it was doom and gloom and all about case number, deaths, and lockdown over there too. Hopefully, Japan speaking out means something has happened to some people behind the scenes. (I just noticed the article was from February 2021, so they were probably trying to fix covid before the Olympics...)
Better than our doctors here in the US, many of whom aren't using their supposed honed analytical skills, or are too afraid to speak up. Or worse, as in the American Medical Association, who have not done anything for American doctors.

Perhaps timing is everything
In part waiting for consensus.
He's not saying something the public is going to be shocked by, but would be waiting on confirmation for.
