I don't believe he has dementia. His meltdowns and missteps are too choreographed, including the prat fall while boarding AF1
I don't see an actor being this good and some months back, he had massive gouges in his hands from being dosed intravenously. Hard to fake those.
Allowing him free rein to be this crass could be part of a plea deal, if he has already been charged, and that's a big 'if'.
