A physician has the capacity to prescribe medications NOT intended for a specific treatment. So you can completely ignore what you heard.
The formulary is what the pharmacy stocks and releases. Don't know why a bureaucratic rule can override a doc's prescription!!
The pharmacist has the duty to prevent the fulfillment of any medication they deem unsafe for the patient. For example, a physician may prescribe a medication that contains an ingredient that is associated with another medication the patient is allergic to, the pharmacist has a duty to first pause the prescription, and call the physician for clarification and provide that information.

You’re talking about contraindicated medications. If you are not taking any other medications that are contraindicated by a prescription a doctor writes for you, the pharmacist has no legal authority to deny that prescription. Your medical history/indications are between you and your doctor.

Pharmacist aren’t filling prescriptions for Ivermectin or HCQ because they have been told not to fill these prescriptions for covid diagnosis; not because they conscientiously object to the medication. So doctors have to lie about the diagnosis and say it’s for scabies or malaria. Then the pharmacist will fill the prescription.

If your doctor or hospital won't answer your questions about their use of Remdesivir, back away from them... slowly. Try not to let them realize you're escaping. Their non-answer is all the answer you need. Other threads have revealed: Hospitals are being paid almost $50,000 per victim (uhh, per case) to 'treat' people with poisons through ventilators. Oh, it's VERY effective (deadly). Trust the numbers. :(

At present, there still are some OTC remedies - maybe at your health food store - that are almost as effective (maybe even AS effective) as Ivermectin. Look for NAC (follow the label directions). Look for Quercetin and vitamin D-3 (again, follow the label directions). Look for vitamin C and THIS IS NOT MEDICAL ADVICE. Seek out a 'naturopathic' medical practitioner. Question them carefully - consider their responses judiciously. Trust your gut.

The right to try law is regulated by the state on what exactly that means.
According to Illinois State law: https://illinois.lawi.us/99th-regular-session-hb-207/ it applies to drugs not approved for general use and manufactures may give that medication/drug but are not required to.
So unfortunately it looks like at least in that state the law allows people to be denied.
People trying it prophylactically would not be terminally ill.
And this is all for our "own good", right?
I'm not stating an opinion. I am simply stating facts. I personally disagree with the state's law and believe people should be able to choose whatever they want in regards to their health as long as it doesn't infringe on the rights of others and they are sincerely pursuing a benefit to their health.
Formulary restriction: it means the hospital is telling the doctor what they can and can't prescribe for all their hospital patients. Kind of like how each fast food place is either Pepsi or Coke, the hospital admins limit what each doctor franchise has access to.
Except it's more complicated and legalistic, and they pretend it's because it helps someone besides their pocketbook.
