As you describe something (like Mandela dying) people will construct pictures in their heads, and facing someone who claims they have a similar picture as memory, it is easy to miscategorise the visualisation the listener has as a memory also.
As far as "Luke I am your Father" - Luke should have agreed to Darths suggestion. "Join me, and together, we can rule the galaxy as father and son!"
Together they could have destroyed the Emperor, THEN Luke could have turned Vader away from the dark side, or defeated him. But meh.
