I spoke with an attorney and they advised me not to state my reason for the religious exemption just that I have one.

You don't even have to say what your religion is.
Religious exemption is not a request, it's a statement of your rights.
That’s exactly what the attorney told me.
In addition it might be wise to counter with something similar to
"..are you asking me questions about my medical history, are you sure you want to do that?..."
"...are you sure you are allowed to ask me that question? I thought prying into a persons medical history was not legal..."
"You aren't my doctor..."
"Um, I'm not comfortable with you asking me about medications or any of my medical history, maybe we should take this to HR..."
Document every encounter where they probe with possible medical questions. Share with lawyer.
