Lulu.com is good for printing not-so-expensive paperbacks.
Just finished printing a few 400 pg books with them, and even full color it was like $20 each for a 9-1/2" x 7-1/2" volume. Plus shipping, but still not bad.
Good question. I've only printed books that don't have printed editions, or that are long out of print. They probably wouldn't actually be so keen about printing a book you can just buy normally like this one.
In my case, the only one I'd be able to buy from the author's site is the PDF since I'm in Japan, so my options are limited. The hardcover would be optimal, considering the subject matter. I'd order it in a minute if I could.
