I guess we should have made her take a random trip in those submarines once in a while. How does ONE person have this kind of responsibilty (power)? F the fine of a million dollars, everything she has should be seized and she be locked up forever.

This is my question, too. There is usually multiple levels of redundancy internally and even independent analysis of those findings
