I had to write mine recently and here is a website that helped me a lot- tons of info on the site like videos explaining the best way to go about writing it and letters to use a reference to guide you. Just make sure the person puts the letter into their own words or it can get declined for plagiarism. https://lc.org/exempt
look up title VII of the civil rights act of 1964
1st amendment guarantees freedom of religion
look to see if any of her state laws talk about no forced vaccination and include that
Some scriptures I referenced were:
Psalms 91:3. 1 Corinthians 6:19-20. Psalm 127:3. Acts 2:4
Someone just posted what she wrote yesterday. Let me see if I can find that. Type in holy spirit in the search and see of something comes up from yesterday. I am using my cell right now and search isn't working for me.
They changed the date from december 8th to get holiday profit. Someone should organize the great american sick out nobody works from december 8th -12th
I, _______, declare religious exemption from the COVID-19 vaccination mandate. Under Title VII of the Civil Rights Act of 1964, I attest to having sincerely held personal religious beliefs which will not permit me to abide by this mandate.
Additionally, I am not willing to describe these personal religious beliefs for the reasonable fear that if others have knowledge of them it may engender discrimination against me and/or my family.
I am aware that reasonable accommodations may have to be made regarding this declaration of religious exemption, and am willing to discuss accommodations that would be necessary and reasonable in order to come to an agreement.
Finally, I expect this letter to remain confidential, and its contents be disclosed on to those for whom there is an express need.
OP, if you use this, I would suggest removing the 'reasonable accommodation' clause as nothing those people do is reasonable and you really can't reason with them. They'll want you tested every day or something. There is no need for an accommodation. The US Constitution guarantees your rights. End of story.
My sister's son-in-law used this website as a guide for writing and ultimately receiving a religious exemption. VA hospital is where he works and it was accepted. Hope you find this helpful. The Healthy American
Also type in Thomas Renz, he is an attorney and he gave some good advice for exemptions. He may have provided an example.

Thanks everyone. I will start sending her these and doing some research. I can't believe this country has come to this. All about "saving lives"
