OP, if you use this, I would suggest removing the 'reasonable accommodation' clause as nothing those people do is reasonable and you really can't reason with them. They'll want you tested every day or something. There is no need for an accommodation. The US Constitution guarantees your rights. End of story.

They changed the date from december 8th to get holiday profit. Someone should organize the great american sick out nobody works from december 8th -12th
