You can file a false affidavit in court testifying that you either have been vaccinated or that you received a negative test result. This is impossible to rebut unless you admit that you lied.
Explain more about this. How does one file an affidavit with the court without a lawsuit?
I think you can just go to the court and tell the clerk you want to swear out an affidavit.
I've never heard of it. Are you referring to a notary public? Otherwise, on what basis would it be filed in a court?
Look, if you are not sure, then it might be better not to say it unless it is confirmed.
An affidavit must be notarized, signed in the presence of witnesses, and the affiant must swear that the facts contained in it are true and correct.
Once the affidavit is witnessed and attested to by a notary public or other official, it holds the force of law and binds the individual to the truthfulness of the information that they have provided.

Instead of providing your employer with a vaccine card, you give them a copy of your affidavit as "proof" of your compliance.
The affidavit won't be filed in court unless you get terminated. Then you file a lawsuit against your employer for firing you for being unjabbed or untested when it "isn't true", and your "proof" is your sworn affidavit. Your affidavit, of course, is perjured testimony, but since it's based off of your own first hand knowledge and nobody else's, it can't be rebutted even if it's an obvious, bald-faced lie.
