I think the idea is that they receive Medicare funds. The government basically said that participating in Medicare was voluntary and if hospitals choose to participate in Medicare they would have to abide by many rules, including having their workers vaccinated. In theory, a private hospital that does not accept Medicare does not have to comply.

The reality is that Medicare payments are a huge significant source of funding and without it they would go bankrupt.
