Having taken the hardest step, the rest (whilst still challenging) are definitely do-able. Lots of good advice on here already, but I would definitely start with..
-No TV
-Exercise
-Healthy Food
Set yourself some short term goals, don't worry about the longer term ones, you aren't ready to set those yet.
Once you've done the above for two weeks, set yourself another 2 weeks worth of goals in addition to the above.
After that first month, if you aren't already in a better place then you probably want to be there. Not trying to be harsh, but even the shit stuff in our lives can often be traced backed to a choice to do, or not do, something if we are honest with ourselves.
Last, but not least, recognise that you are loved and are worthy of love. Get that far and you will never be in this position ever again. A worthy goal, yes?
