Correct, this is coding for the insurance companies to know what to pay the hospitals out for. It’s something a person has to go to school to learn how to read because it’s so convoluted and difficult for the layman to understand. This is America’s medical system.
Modifiers? Are you thinking of CPT? Modifiers are not used for diagnosis coding, only procedures
H is correct, the rules are convoluted, with specific changes for COVID alone. For example, lab results are not used for coding purposes, EXCEPT IN THE CASE OF COVID POSITIVE lab results. We all know how accurate those are.

It's not that difficult. There are modifiers, and that is more burdensome, but really, it's not hard.
