Are kids born to two relatives at significantly increased risk of cognitive and mental deficiencies?

My oldest brother was a product of incest. He has no cognitive issues but he has other physical issues. I don’t know if they are related to the incest or not.

I do know he would marry because he was fearful for any future children he might have.
