Somebody slipped on a banana peel and triggered the 5-level security override codes.

As someone who works at a Nuclear Plant those kind of messages get sent out by accident when we are doing Emergency Plan Drills. Normally done from someone new that is inexperienced using the system. Accidentally send out a real message instead of a drill message. It happens more than you think.
