Call me tinfoil, but I am calling it. Just like that concert last year, I think these are tests to see how well the kill switch works. I think it works pretty much like what was shown in the movie League of Extraordinary Gentlemen.
I'd say the Occam's razor take would be that swimming is probably more strenuous than many are used to so they swim a bit and either over strain things cardiac-wise or kick loose a clot or what have you.
I have nearly killed myself doing some of the yard work that I do. Heart racing and dripping sweat while digging in this heat. If I took the jab, I would have dropped dead a long time ago.
