During many major speeches or press briefings (the few he gives) you will notice he always has what seems to be a candy or something in his mouth.
This will be medicated to get him through the speech/address because of his dementia. It's why he will often have the weird outbursts of either shouting or whispering. It'll be a timed release med, but it will also change his personality in real time.
It's the only way they can get him through the speech in a semi coherent way!
