She is definitely responsible as well, but the doctors and the medical system also bears a huge responsibility. There is plenty of responsibility to go all around.
This is where it get's sticky. One one hand, you could argue that removing perfectly healthy tissue is mutilation, and therefore ethically wrong.
Then, I look at people such as Angela Jolie (yes, she's possibly unbalanced), but she had her breasts removed due to a genetic likelihood of developing breast cancer. So, she made the decision to "hopefully" avoid cancer by removing the tissue, before anything developed. This unfortunately became public (and I would support any decision an adult makes, with pertinent information, for their life).
But to remove tissue on children, for a fashion trend - absolutely not.
