Being able to constantly irritate, annoy, and ridicule the opponent over personal things like their name is the issue.
Trump wiped Rand Paul off the end of the debate stage with one remark about his looks, after Paul went there. "I never commented on his look, and there's a lot of material to work with there." Boom. Done.

I have a pretty good recollection of every Presidential election from 1968 forward. I have never seen anyone else who comes close to President Trump’s ability to end a political opponent’s Presidential aspirations the way he does. And, he does it with a touch of humor.
The simple act giving them a nickname or making an observation about them that communicates their weaknesses more than hours of debate can. It’s analogous to the old “don’t think of a elephant” trick, once he gives them a nickname neither they or the voters seem able to overcome it.
