Dental professional here. When a tooth is accessed in the lower jaw, the swelling would start in the jaw line then drop down into the throat area. Very dangerous because your throat could swell closed. This appears to be a lymphatic problem, his body is fighting an infection, a swollen gland. Good eye though.
I have seen and treated submandibular space infections of odontogenic origin that look like this. It is likely swelling of the submandibular gland (infection), but could be also be lymph node as you point out. Hopefully it is painful to him.
He's got quite the swollen gland under his left jawline. Maybe he has an infected molar.
