I actually first heard the term "out-of-place artifact" in the video game Star Ocean: Till the End of Time, where one plays a minor role in the plot--it was a sphere from which a perpetual supply of clean water flowed forth. Its origins are never explained.

But again, this is my first time hearing this abbreviation. I felt people could use a straight answer for what it meant without having to look it up elsewhere.
