I've waited before in the ER but from what I know if you have chest pains or trouble breathing, or are bleeding out or unconscious etc, they have to take you immediately (in US)
They do the same in Canada this person likely is one of the people who go to the ER for non emergencies. That is what the problem is the hospitals need to make it clear you cant just go there because you have a runny nose or you sneezed. Hospitals are supposed to be for emergencies only but people treat it like a doctors office.
Of course they do order by priority. And yes many of the cases should go to urgent care. But those clinics are not as prevalent, and many, many people lack family doctors.
However, you can be sure that most of the people in line here have a cough.
