I was sceptical about the form but it seems to be referenced on the https://za.usembassy.gov/refugee-admissions-program-for-south-africans/ website so it could be OK.
It is intended to be completed on-line and not printed out, however.

You can do it on your phone purkiss80, had students submit their google forms work through their phones all during covid. Praying that you and yours get out of there.
