I was at a Kroger store recently, one that has a pharmacy attached, and there was a big sign promoting "FREE!" shots for RSV, flu, and COVID... "at no cost to you."
My immediate thought was, "Yeah, no cost to you, EXCEPT your health and your life."

I would never have my pt hold her arm up in the air like that. It contracts the deltoid and makes the muscle harder to inject into. The nurse should've had her rest her arm on the armrest of the chair to relax the muscle.
