There no cartilage in the lobe. Its a fleshy, pliable mound.
If the flesh around the jaw is cut, removed, thus shortened, the lobe would get pulled down and stretched.
Now this can be done on the patient, or this could be done via FACE SWAPPING like the movie Face Off.

The ear itself looks the same to me. Lighting and camera angles can be deceiving, but it looks the same.
