I'm going to say the video is real, but the mics were added after as glitches in the Matrix, as we keep seeing. Specifically the mic is the top layer, in order to be Infront of the actor, right? So then you would need to manually mess with the layers in order to make it go behind his hand...
I think the video is, real, some of the mics are real, but that fat gray deadcat in the lower angle especially - composited in there, intentionally poorly.

Yes there is more than just simple greenscreen happening here.
The microphone layer changes from in front of biden to behind biden.
