Is that because she basically hired and vetted them? I read that she was not on the ship but it makes sense if she gave them the okay to sail without her. I do think it's a little absurd though. This is like when a company hires someone who ends up doing something crazy. The CEO should not be at fault of that unless they hand picked that person.
The captain is always always responsible. This is tradition in both commercial seafaring and military.
A typical tour for a CO is only 18 months. The reason is because the job is fucking brutal and stressful. They rarely get more than a few hours of undisturbed sleep a night because so many things require CO sign off. Switching charts for example. They are responsible for the actions of even the most junior idiots, and are constantly one fuck up away from ending decades of a career.
Civilian side is probably easier, but the captain is still the last word.
