Possibly confirmation bias that's built-in to their reporting structure, the same thing that led to Mao's famine. In that case, everyone had been lying for so long that nobody knew the truth ... until it was too late. Many of their own comms and actions depend on both secrecy and compartmentalization, with very few to tie things together. Just a thought.
