The tip-off is the overly-thick needle. Every needle I've ever gotten was a whole lot thinner than this one. They need to use a thick one, though, so everyone can see it on camera.
Yes, I take a couple medicines with a retractable needle. The video would not play for me, but the needles I use have a loud audible snap when retracting.

The confusing part is that there are now protective syringes which snap the needle off inside. I bet many of the elites have faked it however....

Sometimes they malfunction and the "needle" sticks and doesn't pop out. This has been seen in at least one video I know of.
