Does any of your churches have a good statement written up on this?
I'm looking at being prepared to give one, but I want to make sure that it is the best that it can be.

Psalm 91 in the Bible is what I believe to be important law/literature but most business don't care.

A little advice. The less logical your statement is the better it will be for you. Less is more. A long tirade can be picked apart. Don't mix ANY scientific reasons or understanding with your religious beliefs in your explanation. Also make it general, such as don't defile the body. This keeps you safe from all manner of iterations of vaccines. If you say aborted fetal cells were use they will develop some other form to force you. Be unreasonable with your religious beliefs. If you allow them even an inch they will use it to pry apart your exemption.

Your body is the temple of god. Anyone who defiles the temple will be destroyed. Plenty of verses to back that up. Where does god reside in the physical temple? In the inner sanctum, the holy of holies. What is the holy of holies of your body? Your genes.
Expanding on this a bit. The temple is built around the holy of holies, without it (the place where god resides) there would be no temple. The analogy here should be obvious.
I don't like the term "religious exemption". It implies that you must be part of an organized denomination with specific teachings to be exempt. How about a "spiritual exemption"? I don't need a denominational church or any church to know that injecting aborted fetal cells or man made genes, as apposed to God made genes, into God's temple is sinful. It's an affront and an abomination to God.
I don't think you need to be part of an organized denomination, and it may be 'illegal' for them to ask. Sorry, don't have the reference...something I remember reading somewhere online recently, but with all the stuff going on, I don't remember and didn't think to save.

Simply refuse. You don't need a loophole. I have refused everything they've pushed, masks, socialist distancing and the gene therapy. If you don't stand up for what you believe is right you become part of the problem. I can't count the number of times I've gotten a thumbs up or a "thank you" from someone simply because I refused to kneel for the tyrants. I've visited a relative in the hospital twice without playing their BS mask games. I had a doctor there to wink at me and nod his head because I wouldn't put on a mask when the security guard told me to. The next time I visited no one even tried to make me.
From what I’ve seen, the exemption is usually your own reasoning for why it’s against YOUR religion. Do you need to have a church official sign it?

I looked for what you’re looking for when the vaxx was first being talked about. Couldn’t find anything to go on. Best bet is to compile your own set of verses to go on if you want something to quote, or simply provide your own interpretation. Some good advice in this thread, particularly the comment by u/Newusertopumpmemes about not giving something that can be picked apart.

you do NOT have to have a “church” that you belong to. Whatever religion you follow will count. Religious freedom doesn’t require a waver, no one can ask you to get a note from your pastor for permission.
I bet the Jehovah's witnesses have something online, that's right up their alley, they are famous for refusing blood products, I'm sure they don't want fetal remnants either.
There statement is based upon a principle that I can't in sincerity agree with though. They reject a lot more than I do.

The last time humans changed there DNA God flooded the world. John 8:33 ye shall be in bondage to no man. The moment they didn't wear the mask and I had to I was put in bondage and haven't worn that crap since. I was sent home 3 days in a row. Suspended for 3 days. Just went back in today to get sent home again. I said "your company passed a policy against my religion, don't know what you are supposed to do, but I will see ya tomorrow".
