Interesting.....but in today's polarized and near-totalitarian society, there would be very few companies willing to use this form (it would be accepting liability to do so) and would still require the employee to take a vaccine as a condition of employment.
The only way I could see this working is as part of a large work action or walkout, OR if it was adopted by a large union that threatened a strike unless implemented. But nearly every union these days is chained at the wrist and ankles to the Demwit Party, so that won't happen.
It's a weapon in our arsenal. It makes a clear statement that if they think the vaccine is safe and your being crazy, they shouldn't be afraid to assume liability.
Oh I get the theory, I just don't think many employers will either ACCEPT any liability and (ergo) NOT fill out the form and sign it. There is no way to enforce this form on an employer as a legal document. And if the company is big enough, the legal department might take one look at it and show you the door.
Ya I get that too. Your working on the basis that the worker has no leverage. At my company (we are the largest in the industry) they can't keep fully staffed at any level. You can have leverage and still get screwed since most the time you don't even know you have it.

Well then you may indeed have some leverage. I'm no longer in the work world, I escaped a couple of years ago (it was getting way to PC for me) and I'm doing other things now. So maybe my understanding of the work world is outdated.
I agree. I work for a company with 70,000+ employees. If I refuse an annual flu vaccination, I get walked out the door. They wouldn't give this form a second look.

I live in a right to work state. It's still not worthless. I'm a great worker and an asset to the company. They don't want to fire me, but if they try to force the vaccine on me I'll use this.

Is there one for masks in schools? I see only for the vaccine in colleges. I have other things I'm working on and attending BOE meetings, but that would be even more arsenal to add to my emails and letters.

My boss is pretty cool, so this form is not needed, he's so cool in fact that sometimes he gives me handjobs. Being self-employed is nice sometimes.
