That's complying in my book, unless the card is so obviously fake & provided to prove a point. E.g. first name F last name You.
Also, document forgery is likely a crime & while not submitting to an experimental medical treatment may be defensible, defending forging a federal document is a different story.

Heavy card stock, don't know a precise specification on it, have heard it described simply as 'heavy card stock'.

Do these basic CDC cards have a form number on them. Shouldn't they be in the Government Paper Management Manual? My other question is: CDC is not a Government Organization, so how can their card mean anything?
