I hope that form doesn't put a time limit on the medical issues. It should say the employer who mandates the vaccine accepts the responsibility of any damage to a person's heath that can be traced back to the vaccine now or in the future regardless of whether that person is still employed by the company or whether they were fired with or without cause.
