Go online and print out a blank "vax" card on regular white card stock paper.
Fill in the information:
Name: Nunnya Bidness
Birthdate: 12/31/1905
Signature: (just put a scribble mark and a smiley face. If that "signature" is good enough for election ballot auditors, then it's good enough for vax cards.)
Vax: WWG1WGA
Lot #: MAGA 2020
Date of vax: 12/25/2345
Physician signature: "Julius Caesar"
Basically, just have fun filling in completely made up garbage. :>) Then fold the card or spill a little coffee on it, so it looks like it's been used a few times.
If anyone ever asks to see your vax card, just take it out and show it. My bet is they don't read it anyway (if they can even read, because a lot of high school graduates can't these days). Most will just take a casual glance at it and check that little box in their brain that says "good". You can then go wherever you want.
I would bet that less than 1% of businesses actually will READ the card and understand that it's a complete fake. The other 99% will let you do whatever you want and won't more than even glance at the card.
If you want to get phone "proof" of vax, then find someone else that has the "proof of vax with scan code" on their phone... take a photo of it and keep that photo on your phone. :>) If you're ever asked for "vax passport", then open up your photos on the phone and show that one. They can scan it if they want to, but it will look totally legit.
Keep some seltzer tablets with you and when you get a mask karen or vax nazi harassing you, pop one in your mouth, start drooling, growling and then start staggering towards them.

Every business I know requires a vaccination card, and every bar, restaurant, and venue I've been to this year has required it (Excelsior pass is the digital version, so it's just kept on your phone).
We're wary of strangers who might possibly be rats for the city. Just make a photocopy of a card to show them. The template is all over the internet. It's not our job to determine if your papers are real or not. We just need to see something so we don't get fined. Some places want to see your ID as well. That's actually not legal unless you're purchasing alcohol and appear under 21.

you don't HAVE to move to NY. unless you're military and on orders but I don't think there is much in NYC.
