It would also make sense for the director to be hit if the bullet passed through her and hit him as the directors are often standing next to the camera operators watching how the scene is coming across on camera on an external monitor

That's plausible. It would be great to know if they were filming or rehearsing a scene like that.
Somebody else mentioned that cameras downsight of a gun are unmanned by convention. I guess this wouldn't be the case in your suggestion
